Medicaid Recertification

If you or a family member currently has health insurance through New York State Medicaid, Child Health Plus, or the Essential plan, you may need to take action to renew your health insurance. 

Here are some things you can do now to renew:

  1. Make sure your contact information is up to date with your insurance plan and the New York State of Health, including your current mailing address, phone number, and email address so they can contact you about your health insurance. To update your information, call NY State of Health at 1-955-355-5777 or visit nystateofhealth.ny.gov.
  2. Sign up for text alerts from NY State of Health for latest insurance updates. To subscribe, text START to 1-866-988-0327.
  3. When it is time to renew your health insurance, you will receive a letter from NY State of Health or a renewal packet with information about your coverage. Fill out and submit your renewal form.

  • In order to assist you in determining whether Maimonides accepts your insurance plan, please see the list of insurance plans that we participate with below. Please note: some health plans use smaller networks for certain products they offer so it is important to check whether Maimonides participates in the specific plan you are covered by. You may also contact a Maimonides Patient Pricing Transparency Unit at 718-283-CARE (2273) to verify if your insurance is accepted
  • Maimonides standard charges for the items and services it provides are available by calling our Patient Pricing Transparency Unit at 718-283-CARE (2273)
  • If you do not have health insurance, you may be eligible for financial assistance with the hospital portion of your bill. Information about the Maimonides financial assistance program is available on the Financial Assistance page or by calling the Maimonides Patient Financial Services Office at 718-283-7790
  • It is also important for you to know that the physician services you receive in the hospital are not included in the hospital’s charges. Physicians who provide services at the hospital may be independent physicians or they may be employed by the hospital. Physicians bill for their services separately and, even if employed by the hospital, may not participate in the same health plans as the hospital. You should check with the physician arranging for your hospital services to determine the plans in which that physician participates.
  • Plan participation information for physicians employed by the hospital can be found on their pages in the Physician DirectoryBecause physicians’ insurance participation can change, the insurance information on this page may not always be up-to-date. Please contact the physician directly to obtain the most up-to-date insurance information
  • Maimonides Medical Center contracts with AABP, LLP anesthesia services for inpatient and outpatient anesthesia services. You may contact them directly at 718-283-8816 or visit their website, www.aabpmed.com/insurance, to find out the health plans in which they participate
  • You should also check with the physician arranging for your hospital services to determine whether the services of any other physicians will be required for your care. Your physician can provide you with the name, practice name, mailing address, and telephone number of any physicians whose services may be needed. Your physician will also be able to tell you whether the services of any physicians employed or contracted by Maimonides Medical Center are likely to be needed, such as anesthesiologists, radiologists, and pathologists. You should contact these providers directly to find out the health plans in which they participate
