FollowMyHealth® Patient Portal

The FollowMyHealth® Patient Portal works with your Maimonides account to give you more control over your healthcare. Access your personal medical records from Maimonides and other medical providers on your computer or mobile device.

FollowMyHealth® is used by hospitals, doctor’s offices, and health networks across the country to provide patients with their personal health records, which enables you to take a proactive role in managing your care. FollowMyHealth® is available online 24/7 via any computer, mobile device, tablet, or smartphone.

Need a FollowMyHealth Account?

An invitation is required to connect FollowMyHealth to your Maimonides health information. Maimonides patients typically receive an email invitation when they have a visit or procedure at Maimonides. Look for an email invitation from [email protected] with subject “Invitation for connecting to Maimonides Medical Center”. Can’t find your email invitation? Proxy access for child patients

If you are the parent or legal guardian of a Maimonides patient who is under the age of 12, you can request proxy access to your child’s health information in the FollowMyHealth® patient portal. Proxy access enables you to view some of your child’s health information in the portal and, in some instances, communicate through the portal with your child’s healthcare team. When your child turns 12, you will not have access to any new information in your child’s portal.

At this time, proxy access is not available for children between the ages of 12- 18.

Proxy access for adult patients

Through the FollowMyHealth® patient portal, registered users can request online access to medical information for an adult in their care who has impaired decision-making abilities. Proxy access enables you to view the adult patient’s health information in the portal and, in some instances, communicate through the portal with the adult patient’s healthcare team.

Access FollowMyHealth® on a mobile device

You can install the free FollowMyHealth® app from the iTunes App Store or Google Play Store. Once you have the app, you can choose to specify certain notifications to be sent via text message to your mobile phone. Text messages will not contain protected health information.
