Site brings primary care, specialty services, and care coordination under one roof, serving Midwood, Flatbush, and surrounding communities
Brooklyn, NY (April 16, 2026) — Today Maimonides Health announced the opening of the new Maimonides Doctors Nostrand Avenue Multispecialty Center. Continuing Maimonides’ commitment to expanding access to high-quality, community-based care, the new multispecialty care center is designed to bring coordinated primary and specialty services closer to home for Brooklyn residents.
The expanded center located at 2316 Nostrand Avenue builds on a long-standing primary care practice to now offer on-site specialty care, diagnostic services, and care coordination, making it easier for patients to access the full spectrum of care in one convenient location.
Serving the diverse communities of Midwood, Flatbush, Little Haiti, and Flatlands, the new center reflects Maimonides’ broader initiative to meet patients where they are and reduce barriers to care. This new location offers providers fluent in Spanish, French, Russian, and French Creole, as well as English.
“Our existing patients have come to know and trust the quality primary and preventive care they’ve received here for more than a decade,” said Cary Daniel, MD, Primary Care Physician. “This new chapter brings these services under one roof together with on-site specialty care from Maimonides’ expert physicians across multiple disciplines, providing patients greater convenience to address their unique medical needs.”
Primary care services include preventive screenings, chronic disease management, immunizations, mental health support, sexual health services, and same-day or next-day sick visits. The site also offers women’s health services, lifestyle and weight management counseling, and care for adults and seniors. In addition, patients have access to on-site specialty care and diagnostic services.
Patients can also access a growing range of on-site specialties, including:

- Endocrinology
- Gastroenterology
- Infectious Disease
- Rheumatology
- Urology
With on-site lab work, EKGs, and coordinated referrals for imaging, patients can often complete key aspects of their care in a single visit, reducing delays and improving follow-up.
“Primary care is where we have the greatest opportunity to prevent disease and manage chronic conditions before they become crises,” said Sophia Kostelanetz, MD, MPH, Site Medical Director. “By working closely with specialists in the same setting, we can intervene earlier and help patients stay healthier over time.”
Beyond clinical care, patients will also have access to a Community Care of Brooklyn (CCB) Navigator, who helps connect individuals to resources addressing social needs such as food access, housing, mental health, and medication support.

The Nostrand Avenue site offers short wait times, extended weekday hours, and multilingual providers, helping ensure care is accessible to Brooklyn’s diverse communities. The site is fully connected to the broader Maimonides Health system, ensuring seamless coordination with hospital-based and advanced specialty services when needed. It also strengthens continuity of care for patients of Maimonides Midwood Community Hospital, creating a seamless connection between emergency, primary, and specialty services within the same neighborhood and same health system.
