For referring providers, fragmented care is a persistent challenge. Patients often need separate appointments across multiple locations for primary care, specialist follow-up, and women’s health services. This leads to delayed care and gaps in coordination.
Maimonides Health is changing that with the new Maimonides Adult and Pediatric Care—7th Avenue at 6323 7th Avenue in Brooklyn. The new multispecialty outpatient hub unites adult primary care, pediatric primary care, obstetrics and gynecology, endocrinology, urology, and nutrition counseling under one roof for patients in Borough Park, Windsor Terrace, Kensington, and the surrounding communities of southern Brooklyn, with neurology services joining the practice soon.
Multidisciplinary care built for better outcomes
The 7th Avenue hub is built around the principle that connected care produces better outcomes. When a patient’s primary care provider identifies a need for specialist consultation—whether in gynecology, endocrinology, or urology—that follow-up appointment can happen within the same practice and with the same care team in close communication.
The center supports timely access at every step. Same-day appointments are available across services, and patients can self-schedule online through MaimoCare, reducing wait times and the administrative burden on referring providers. The office also features a private lactation room and a dedicated children’s playroom, supporting families navigating multiple care needs at once.
Addressing unmet women’s health needs
Maimonides developed the 7th Avenue hub in direct response to conversations with patients and community leaders who identified a gap in OB/GYN care across Brooklyn. The women’s health practice offers a full continuum of care, including:
- Annual well-woman visits and preventive screenings
- Prenatal and postpartum care
- Family planning and contraception
- Menopause management
- Cervical cancer screenings and breast health evaluations
- STI testing and treatment
- Urogynecology services, health education, and counseling
“Women are often the last ones to take care of themselves. They’re managing their families, their jobs, their parents—and their own health keeps getting pushed to the back. This practice is designed to make it a little easier to say yes to your own care, because everything you need is here in one place,” says Scott Chudnoff, MD, Chair of Obstetrics and Gynecology at Maimonides Medical Center.
Growing the primary care foundation
Alongside specialty services, the 7th Avenue hub meaningfully expands Maimonides’ outpatient primary care capacity. This is a priority as demand for accessible primary care in Brooklyn continues to grow.
“Primary care is the foundation of population health, and this new center allows us to build stronger medical homes for more Brooklyn residents, with the specialist support patients need already on-site,” says Matthew A. Weissman, MD, MBA, Chair of the Department of Medicine at Maimonides Medical Center. “This simplified connection to specialty care makes it easy for patients to benefit from collaborative care across disciplines, addressing their concerns quickly and driving positive outcomes and experiences.”
Board-certified physicians in internal medicine, pediatrics, and obstetrics and gynecology staff the center. Certified nurse-midwives and clinical specialists in endocrinology and urology are also an important part of the overall comprehensive care model. Each specialist brings deep clinical expertise to a community setting.
Serving Brooklyn’s diverse communities
Maimonides has long been defined by its commitment to Brooklyn’s diverse neighborhoods, and the 7th Avenue hub reflects that mission. Providers and staff speak English, Spanish, Cantonese, Mandarin, Russian, Ukrainian, Armenian, Sinhala, and Tamil. That diversity helps ensure culturally sensitive, patient-centered care for the wide range of communities the center serves.
The location at 6323 7th Avenue is accessible via multiple public transportation options including the N and R subways and B9, B16, and B70 bus routes, in addition to proximity to the Gowanus Expressway. Affordable parking is available one block away at Maimonides Cancer Center on 8th Avenue.
