Delivering informed community services

Maimonides Health conducts regular community health assessments to inform the community services that our organization delivers. Through the assessment process, we gather input from community-based organizations, local residents, and primary care providers, in addition to analyzing public health data.

Every three years, Maimonides publishes a Community Health Needs Assessment and Community Service Plan that aligns with the New York State Department of Health Prevention Agenda priorities. This report is approved by our Board of Trustees and filed with the New York State Department of Health. By conducting and releasing this report, we strive to better Brooklyn.
